What should I bring to my appointment?
  • ID card/Driver’s License
  • Insurance card (if you have insurance)
    Previous records (if you are a new patient)
  • All medications that you are taking
  • Payment (Cash or Card)
When should I arrive for my appointment?
Please arrive 15 minutes prior to your scheduled appointment.
Cancellation policy
We understand that life happens. If you need to cancel your appointment, please endeavor to cancel at least 24 hours in advance. You may be subject to a fee if you cancel less than 24 hours in advance.
Late policy
Your appointment may be rescheduled if you are more than 15 minutes late.
Emergency issues
If you have a true emergency, please call 911 and go to the nearest ER.
Medication Refills

If you have not been seen in the clinic for 6 months or more, please call the clinic to schedule an appointment for any medication refills, except if the refills have already been approved by your provider.

New Patient

Click the link “New Patient” prior to your scheduled appointment. If you are unable to do so, please arrive about 25 minutes prior to your appointment to allow time to fill out the forms.

Paperwork

We will only fill out paperwork for our already established patients. Please allow at least 1 week for completion of paperwork.
