What is abnormal uterine bleeding?
"Abnormal uterine bleeding" refers to any bleeding pattern that falls outside the expected range of your menstrual cycle. Because every woman’s cycle is unique, your provider evaluates bleeding based on your individual symptoms and patterns—but there are general guidelines that help determine what is typical.
Your provider will assess:
- Cycle frequency: A normal cycle generally ranges from 21 to 35 days. Significant fluctuations or cycles that consistently fall outside this range may indicate a hormonal imbalance or other condition.
- Duration of bleeding: Most periods last 4–5 days. Bleeding that lasts fewer than 2 days or continues for longer than 7 days warrants evaluation.
- Amount of bleeding: On average, women lose about 2 tablespoons of blood during a period. Losing more than 5 tablespoons may indicate heavy menstrual bleeding. Noting how often you soak through pads or tampons can help your provider estimate blood loss accurately.
Keeping a record of your cycles—including timing, flow, and symptoms—can greatly assist in diagnosing the issue.
What causes abnormal uterine bleeding?
Abnormal bleeding can result from several different factors. Many causes are related to disruptions in the balance of estrogen and progesterone, the hormones that regulate ovulation and menstruation. Hormonal fluctuations are especially common during puberty and perimenopause.
Possible causes include:
- Hormonal birth control methods (pills, implants, IUDs)
- Rapid or significant weight changes
- Chronic stress
- Uterine fibroids or polyps
- Adenomyosis
Less common but more serious causes include:
- Bleeding disorders
- Anticoagulant medications
- Cervical, uterine, or endometrial cancer
- Endometrial or cervical infections
- Thyroid, adrenal, liver, or kidney disease
Important: Abnormal bleeding during pregnancy or after menopause should be evaluated immediately. These situations can indicate urgent medical concerns.
How is abnormal uterine bleeding treated?
